Insurance

We will be happy to review your healthcare benefits and coverage, though there is no guarantee your insurance provider will pay for your treatment. We ask that you bring up-to-date insurance information with you at each visit, and if you do have coverage, we will file a claim for you. Any remaining balance after insurance will need to be paid in full within 30 days, unless you have arranged a payment plan with our billing department.

Co-payments & Non-Covered Services

If you have healthcare coverage, all co-payments, deductibles, and co-insurance fees specified by your plan are due at the time service is rendered. If you do not have insurance, or if you elect to undergo treatment that is not covered by your provider, you will be required to pay in full at the time of service.

All payments can be made with cash, personal checks, and most major credit cards (Discover, Mastercard, Visa, and American Express). In the event you are not able to make the required payment at the time of service, your visit may be rescheduled for a later date.

Referrals from Primary Care Physicians

Some insurance companies require a referral from your primary care physician (PCP) in order to be seen at our practice. If this is the case, we will gladly fax the initial request for you, but you will be responsible for ensuring we have received the referral from your PCP before you come in for your appointment. If we have not yet received the referral—or if the original referral has expired—your visit may be rescheduled until a valid PCP referral is acquired.

Appointment Cancellations / No Shows

Should you need to cancel your appointment and/or reschedule for a later date, we ask that you provide us with a minimum of 48 hours notice. Failure to notify us at least 48 hours in advance—or if you are a “no-show” for your visit—may result in a $25 fee that will need to be paid before another appointment can be booked.

Late Arrivals

Maintaining a strict schedule is critical to the efficiency and efficacy of care we are able to provide. For this reason, we request that you arrive on time for your appointment, if not early. Anyone arriving more than 15 minutes late may have to wait or reschedule to be seen, as on-time patients are always given priority.

Outside Fees

All testing referred to a third-party facility will be billed independently, and any payments and/or questions should be handled with the facility of service. This includes labwork, pathology tests, and various other diagnostic testing.
