Patient Forms
In preparation for your appointment, our office will email you the New Patient Intake Forms to fill out online and submit before your visit. All documents should be submitted prior to your appointment to minimize any delay on the day of your visit.

Handicapped parking and accessibility are on the north side of the building.
Directions from I-95:
Take Exit 79 East onto PGA Blvd, then continue for about 0.3 miles and turn right onto RCA Blvd. Continue for 1 ½ blocks, and you'll find the first building on the right – Gardens Cosmetic Center – Suite 101.
Directions from Florida’s Turnpike:
Take Exit 109 onto PGA Blvd and turn left. Continue east for about 2 miles, and after passing the I-95 entrance, make a right turn onto RCA Blvd (just after the Mobil gas station). Continue for 1 ½ blocks, and the first building on the right is Gardens Cosmetic Center – Suite 101.
Directions from Alternate A1A:
Head West on RCA Blvd to the stop sign, then turn right and continue for one block. Our office will be on the left-hand side – Gardens Cosmetic Center – Suite 101.
