For your convenience, we have provided several patient resources on this page that can help make your experience with us as pleasant and seamless as possible. Patient Forms

In preparation for your appointment, our office will email you the New Patient Intake Forms in advance to fill out online and submit prior to your visit. You may also access the online forms here New Patient Intake Forms – Printable. We encourage you to provide our office with your intake forms prior to your appointment to minimize any delay on the day of your visit.

Please be sure to bring your photo ID, insurance cards, a list of current medications, discs with pertinent CT or MRI imaging studies, and any other relevant medical records to your visit. We will collect an office copay, if applicable, at the time of the visit, as well as any portion of your insurance deductible that has not been met. For all cosmetic appointments, we will collect the consultation fee during the check-in process.

Out of Town Patients

Because of the beautiful location and unique nature of our practice, we often see patients from around the country as well as from around the world. Doctors from many countries have shadowed our doctor, and as a result, an international presence has been established. Cosmetic eyelid surgery, trauma, and eyelid complications following cosmetic surgery are the most common reasons for out-of-town patients to seek a consultation with our eyelid expert.

If you are coming to us from out-of-town, we are happy to assist you in your travel needs. We recommend you come to town the day before your surgery and plan on staying here through the early post-operative period, if possible. We recommend you travel with a companion who can assist in your recovery; however, if this is not possible, our office can find a nurse to assist you while you recover.
